This booking covers the following ORI#: FL921250Z. For all mobile appointments, the price includes 10 miles travel to your location from the nearest office, Tampa (33619) or Miami (33130). All additional miles will be invoiced at current IRS mileage rate. Please be sure to indicate the full address (including apartment or suite numbers) on the appointment form to ensure that you are quoted for mileage accurately. The price includes taxes. There will be no refunds after your fingerprints have been submitted.
